Attention all other company licensees: We will mail each company a “hard copy” of their license renewal application in August. All renewal applications will also be available from this page for download from September 1 through January 31. Simply click on the type of form you need in the list below when available. These forms can be completed, but not submitted, on line. To be considered on time, renewal applications must be postmarked, or have proof of shipper pickup, no later than December 1.
